He is available for QME evaluations and Agreed Medical Evaluations.
If necessary, a DISABILITY STATUS form is faxed immediately after the evaluation to notify the
employer of the medical status/AOE-COE and whether a disability exists.
A comprehensive report is provided with a detailed analysis containing substantial medical evidence
in the appropriate format accepted by the WORKERS' COMPENSATION REFORM ACT OF SB 899.
